Responsibilities
No After Hours On-Call Requirement! 1-2 times a month, expected to
work 1 Saturday or Sunday.At CommonSpirit Health at Home, our

perform comprehensive assessments, collaborating seamlessly with
our multi-disciplinary team of experts, including PT, OT, ST, MSW,
and paraprofessionals. Together, we will work passionately to
achieve the heartfelt outcomes and goals of our patients, leading
them on a journey of healing and hope.Supervise your caseload,
nurturing the growth and development of LPNs and Home Health Aides,
as we cultivate a team of caregivers. In your role as a facilitator
of care coordination, we celebrate the unity that arises when PRN
staff steps in, ensuring seamless support for your patients. We

Qualifications
Pay Range
$31.04 - $45.01 /hour
