Schedule/Availability Requirements:Preferred schedule for locum:Friday: OR: 07:00- 15:00 1st Call: 15:00 - 07:00Saturday: OR: 07:00 - 11:00 (OR may be extended depending on volumes) 1st Call: 11:00 - 07:00Sunday: OR: 07:00 - 11:00 (OR may be extended depending on volumes) 1st Call: 11:00 - 07:00 In addition to weekend coverage, the following dates are also needed:

October 30 Nov 3; 7a 7a
Nov 25 Dec 1; 7a 7a
Dec 16 Jan 5; 7a 7aFacility Location: CHI Health Good SamaritanSetting: HospitalDuties: General anesthesiaFTE: n/aRequired Procedures: n/aEMR: EpicSupport Staff: n/aCredentialing Information: Timeframe: 90 days - need to fully Health Good Samaritan & with CHI Health PartnersRequirements:Credentialing with other facilities? Certification Requirements
